For direct Zurich customers please note that submission of this form does not automatically change the cover under your policy. One of our team will contact you shortly to confirm that change. Changes to the policy can only be made by the policyholder or delegated person.
Please note, Zurich will need consent from the policyholder by phone or in writing before we can deal with a delegated person.
